Four Easy Steps to Save

To receive your rebate by mail, follow these simple steps:

1. Purchase two or more qualifying American Standard products totaling at least $125 on the same receipt between 9/12/11 and 12/6/11.
2. Complete this form. (Handwriting must be legible for your submission to be processed)
3. Enclose a copy of the store identified sales receipt with the purchase prices circled.
4. Enclose the original UPC symbol cut from the product packaging for ALL products purchased. (When cutting out the UPC symbol, please be sure to include both the barcode and numbers.)
